Palliative Care

Palliative care at home allows you to end your journey in comfortable, familiar surroundings.

With the advances in modern medicine, most of us are living longer and now die an expected death. This provides time for us to plan and communicate with loved ones how we wish to be cared for.

Despite this, studies have shown that most of us are not dying the way we wish to and are continuing to die in the clinical setting of a hospital. We want to ensure that this is not the case and that quality of life continues for as long as possible in the security of your home.

Through our palliative care services, we work with you, your loved ones and multi-disciplinary health teams, including palliative care specialists, allied health practitioners and your doctor. We provide you and your family with the support and holistic care needed to achieve your goals.

Adaptive Nursing is here to help support you and your loved ones in the comfort and familiarity of your home. We can help with everything you need, from medication to cleanliness and hygiene.

We understand that each person is unique and we accommodate individual preferences with dignity and respect. We follow essential principles for palliative nursing care, as we know that this journey can be filled with fear, trepidation and unanswered questions about dying. We support you and your family during this difficult process. 

What are the principles of palliative care?

The primary principles of palliative care are to ease suffering or pain, maximize a patient’s quality of life, provide psychological and social support to the patient and their family, and allow patients to end their life’s journey in comfort and dignity.

According to the World Health Organization, palliative care should:

​

·       Provide relief from pain and other distressing symptoms

·       Affirm life and regard dying as a normal process

·       Intend neither to hasten nor postpone death

·       Integrate the psychological and spiritual aspects of patient care

·       Offer a support system to help patients live as actively as possible 

·       Help the family cope during a patient’s illness and in their own bereavement

·       Use a team approach to address the needs of patients and their families

·       Enhance quality of life, which sometimes also positively influences the course of illness

·       Be applied early in the course of a terminal illness, in conjunction with other therapies and treatments intended to prolong life, such as chemotherapy or radiation therapy.

Does palliative care mean death?

​

Palliative care doesn’t necessarily mean death. Palliative care is only recommended when a patient is diagnosed with a life-threatening illness, but death isn’t always inevitable. 

The goal is to provide support and give you the best quality of life possible as you battle an illness, and in the case of death, to make that transition as painless and dignified as possible.

The Adaptive Nursing service

Palliative care is available at private home care fees for the type of services needed – see schedule of fees outline

If you are in receipt of a Home Care package, you can advise your service provider that you want Adaptive Nursing to provide the support needed. If you don't have a Home Care Package (HCP) and think you may be eligible for one, contact us and we will help you with a My Aged Care (MAC)  assessment, to determine your eligibility status. 

​

Additional funding support is also available via Palliative Services QLD per GP – specialist referral for clinical palliative service support. Adaptive Nursing can advise you and your family of the choices available for medical care, helping you to decide on hospital, hospice or home care.

​

We can also help you develop an Advance Health Care Directive (AHCD), so your health choices and treatment protocols are documented and referred to in an event of a medical emergency (See Travel and Nursing Consultant information).
