Supports for NDIS Participants To Complete Your Everyday Tasks and Activities: 
Includes Supported Independent Living (SIL) or Short-Medium Term Accommodation (STA) for respite care. 

To achieve your lifestyle and health goals, Adaptive Nursing has a team of compassionate and trained care workers,  to provide supports (capacity for up to 24/7), to empower you, on life's journey! We focus on supporting you and your family to achieve independence so you can love the life you lead. We do this by developing individualised and comprehensive plans that are adaptive to suit your unique care needs. We have onsite Registered Nurse  available 24/7 for SIL/STA support, for high specialised nursing care, when required.

It begins with a consult. We will take the time to discuss what your needs, goals and dreams are. We will consult with your NDIS coordinator and plan manager, to ensure supports are personalised and within your budget constraints.

Rates, for services, are priced using the NDIS price guide, tailored to suit you,  written into the Terms of Agreement, including gaining your consent or that of your guardian or delegate to gather information and deliver services.

Choice, Freedom and Control

Adaptive Nursing works closely with the National Disability Insurance Scheme (NDIS) which entitles people with a permanent and significant disability under the age of 65 to full funding for any reasonable and necessary support needs related to their disability (subject to certain restrictions). 

Funding is allocated to the individual, and the individual or their guardian chooses their preferred provider. Funding is available for early intervention support for children with developmental delays and those with intellectual, physical, sensory and psycho-social disabilities.

The aim is to enhance your quality of life, help you maintain your independence in the home and give choice, control and freedom. Fees payable for service provision are based on your eligibility, your budget plan and the NDIS Provider Price Guide.

Adaptive Nursing has well-established relationships with several NDIS plan managers and coordinators to ensure that we can provide quality care, while promoting independent living for each client.

Our Disability Support Services

Do you need a helping hand or clinical care? We can provide both in one home visit. The advantages of having Adaptive Nursing provide you with care support is that you benefit from the professional knowledge and expertise of an RN at care worker rates for non-clinical care.

 

If you’re in need of clinical support, such as wound care or catheter changes, we can provide a dual service.

For example, if you receive 1 hour of social support and 1 hour of clinical care in the one home visit, these dual tasks are billed at separate NDIS claiming codes as per your NDIS budget plan.

Adaptive Nursing offers a wide range of services:

  • PEG feeds, home meal preparation and feeding

  • Medication administration as prescribed by GP / health specialist

Health and wellbeing

​

Maintaining your health and wellbeing is important, so you can live your life your way. We can provide you with the clinical care prescribed by your GP and attend to any clinical interventions in the privacy of your own home. We can attend doctor's appointments with you, liaise with specialists involved in your care (such as an occupational therapist, physiotherapist, or psychologist) and help build your care plan. Social Support and Companionship 

​

It's not uncommon for people to experience feelings of isolation, especially when away from friends and relatives.  We will provide you with caring and sincere companionship when it's needed most.

 

Meal Preparation and Groceries

​

Meal preparation and grocery shopping are essential activities that can sometimes be a challenge for people to complete independently. We are here to help you with these essential daily tasks.

​

Life skills Training

​

Do you need help and support with relevant skills training to enhance your level of independence in the home and community? With a focus on teaching transferable skills, we will help you work towards greater independence without you feeling like a client or patient.

​

Developing a service agreement

​

Adaptive Nursing can provide you with a service agreement which explains your rights and responsibilities and our terms and conditions. We will work with you to select your preferred times and days for care and support. We will arrange with your provider how to pay for your support. If we can’t provide the services you want, we will even help you locate an alternative provider.

 

Changing or cancelling a booked shift

​

Adaptive Nursing require a minimum notice of 48 hours should you change or cancel a service, otherwise we reserve the right to charge the full fee applicable.
